TO BECOME A CERTIFIED COMMUNITY MEMBER

Step 1: Please fill out the Proposed Beneficiary Criteria Enrollment Application Form which is located in the links below.

Step 2: Attach photo copies of supporting documents for proof – Long Form Birth Certificate or Baptismal Certificate.

Step 3. If you have any issues filling out the Proposed Beneficiary Criteria Enrollment Application, please feel free to contact Denise Chaput (613) 732-7777 to discuss it over the phone or set-up an appointment.

Step 4: The whole package can be dropped off at our office or sent in either by Canada Post or by email.

Please note this is the only enrollment form we will accept going forward.

Upon receiving a letter from the Enrollment Officer for passing the Proposed Beneficiary Criteria then you can obtain a Community Photo ID Card. We do not issue status cards.

Only submit a photo if above the age of 18 and if you passed the Proposed Beneficiary Criteria.

New Community Identification Cards

Issuing New Community Identification Cards To Certified Members Only

We need you to send us up-to-date photos for the New Community Identification Cards.

Step 1. Stand against a light-colored background (Wall or Door).

Step 2. Ask a family member or a friend to take a clear photo of you from the shoulders up like a passport photo with a cellphone.

Step 3. Send photos with names by email to Denise Chaput.

If you live within or visiting our vicinity, you can stop by and Denise Chaput will take your photo and make-up your new Community ID Card to take home same day.

When sending in multiple photos, please give us the person’s name for each photo. And if you want the new Community ID Card mailed out to you please provide us with your mailing address, email and phone number. This way we can update our Data Base and you will receive your new Community ID Card.
